Insurance*
- We are mostly a fee-for-service practice. 
- Michelle Magid and Paula Storrer are currently not accepting insurance. 
- Anna Gumble, Justin Cullers, Isadora Fox, and Anna Robinson are accepting a limited amount of 
 Blue Cross Blue Shield PPO , United Behavioral PPO, and Seton/Ascension Healthcare patients.
- Ketamine Assisted Psychotherapy is not covered by insurance. 
Other Professional Services
- Other professional services (i.e., report writing, filling out forms, consultations, record review, phone calls greater that 5 minutes) are prorated at $400 per hour for all providers. 
- Stimulant refils outside of appointments is $35 per script. 
- We do not provide legal/forensic psychiatric services. If we are mandated to do legal/forensic work (i.e. testifying in court, rendering an opinion, report writing, filling out forms, consultations, record review, phone calls), fees are prorated at $660 per hour for all providers. 
*We understand that insurance can be confusing. If you are BlueCross Blue Shield PPO, United Healthcare PPO, or Seton/Ascension Healthcare patient, we will submit the claim to your insurance company. Patients are responsible for knowing their coverage and making sure their insurance has not lapsed. Whatever insurance does not pay for the visit (i.e., copays, deductibles, inactive insurance policies, policy exclusions for mental health, and HMO plans), patients are responsible for the remaining balance.
We are not a contracted provider with any other insurance plans and we do not bill other insurances directly. Our office will provide a receipt so that you can submit for out-of-network visits.
